Reduction of SSIs through real-time feedback

2. OR Monitoring System: Wireless software- and sensorsystem

As much as 5% of all patients undergoing a surgical procedure will develop a surgical site infection (SSI), making SSI the second most common health care associated infection (HAI). This complication of surgical procedures causes patient morbidity, mortality and results in a considerable financial burden for healthcare providers. The parameters influencing the occurrence of SSIs have been subdivided into behavioural and environmental indicators:

 

Behavioural parameters:

  • OR door movements
  • Antibiotic prophylaxis
  • Pre-operative hair removal
  • Perioperative normothermia

Environmental parameters:

  • Room temperature
  • Air pressure
  • Humidity
  • Temperature difference

The effect on the reduction of SSIs has been proven for all of these parameters. To reduce the prevalence of SSIs, it is necessary to register and measure these parameters and to incorporate the interventions into daily practice. The OR Monitoring System combines all three aspects in one application.

 

In co-operation with several renowned Dutch hospitals, including the Erasmus MC: University Medical Centre Rotterdam and VU University Medical Centre, NewCompliance has developed the revolutionary OR Monitoring System. 

This system registers, analyzes and provides real-time feedback on the behavioural and environmental SSI prevention indicators in the OR. The OR Monitoring System increases awareness among surgical team members, regarding the impact of their behaviouron the occurrence of SSIs. Due to the real-time feedback that is being provided in the OR, surgical team members are motivated to comply with the infection control and prevention guidelines. This will contribute to a higher level of patient safety.

3. Patient Safety Score: visualisation of compliance with all SSI prevention indicators

 

At the end of a surgical procedure the OR Monitoring System generates a ‘Patient Safety Report’.

A score is awarded to the surgical team, based upon the level of compliance with the behavioural and environmental standards.The delivered quality and the level of patient safety during the completed surgical procedure are instantly visualised. The impact of this new method of real-time reporting per procedure and per patient is enormous. After implementation in the Dutch hospital Havenziekenhuis (Rotterdam) a significant positive influence on the infection prevention behavior of the OR team was measured. Because transparency is becoming more important, this specific hospital even plans to include the‘Patient Safety Report’ in the patient record.

4. ‘Overview Monitor’: Monitoring and clearing ORs remotely

Through an ‘Overview Monitor’ all activities and several SSI prevention indicators of all ORs are visible on one display. This provides a clear overview of what is happening at any time in the ORs. During a surgical procedure the air quality parameters, the number of OR door movements and the time of incision are shown on the ‘Overview Monitor’. Additionally, the type of surgical procedure and the surgeon are displayed. The OR manager is able to access this overview and monitor the ORs remotely. When all parameters are within the standards, the OR manager can use the software to clear the ORs for surgery.

 

5. Communication and integration with existing systems

 

The OR Monitoring System can easily be connected to and integrated with present data management systems. Communications are realised through a.o. HL-7 or http-requests.

Direct communication between systems enables the hospital to use NewCompliance's OR Monitoring System without the need to register data twice.


Air quality and door movements are measured through NewCompliance's wireless sensors. These sensors adhere to the medical standards IEC 60.601-1&2. Measurement of environmental parameters is also possible through an integration with the existing building management system (BMS).
